Description:

Our client, Calderdale Royal Hospital, are looking for a Team Leader to join their Team.

The role of Team Leader is to deputise for the Team Manager and assist in managing a Community team. The role is to provide professional support to the team and to support the Manager to ensure that resources, including staffing resources are effectively co-ordinated, deployed and managed. Whilst ensuring that the provision of care to meet assessed needs and outcomes is in accordance with statutory responsibility, policy and team objectives and service priorities and to work to safeguard vulnerable adults. The post holder will manage a caseload of complex cases.

To support the hospital discharge team and Intermediate care team with safe and timely discharges covering Calderdale Royal Hospital and Huddersfield Royal Infirmary. Must be able to attend the office at least 2-3 days per week.

Ensure that effective systems are in place to screen referrals against eligibility criteria:
To work with partners to ensure that referrals are dealt with by the creative deployment of resources both in terms of staffing and other options.
Prioritise and allocate work to appropriate staff based on the complexity and urgency of a service request. To utilise the resources of the whole team and other skills within the overall service.
Ensure the effective management and deployment of resources within budgets to meet agreed policy objectives and service priorities. To communicate effectively with the team about the effective management of budgets.
Responsible for ensuring that the review and evaluation of Support Plans are carried out in accordance with practice guidelines, and that appropriate adjustments are made to Support Plans to reflect changing needs.
Responsible for ensuring a systematic approach to the assessment planning, implementation and review of health needs in accordance with policies and procedures.
Advise and consult with the appropriate Team Manager & Service Manager on expenditure for community care services which exceed current guidelines.
Ensure that staff have fully informed and involved users and carers in the process of assessment and in agreeing personal budgets, packages of care, and to ensure copies of assessment and care plans have been provided by staff.
Promote the professional development of individual members of staff through regular supervision, identify training opportunities, work in accordance with agreed outcomes in Performance Appraisal Scheme interviews and assist qualified nurses and social workers to maintain their professional registration.
Responsible for own professional development in accordance with SW England registration requirements as a qualified Social Worker.
Assist the Team Manager in ensuring that the team's work is adequately covered in staff absence and to take responsibility, as required, for covering the work of the team manager in their absence and to monitor/manage sickness absence in accordance with Council policies.Benefits of working with Remedy
