We are searching for a dynamic individual to take on the unique role of Supported Pathway Officer. Assist residents in securing their next stage housing, providing resettlement support to move into and sustain their new home successfully. To provide appropriate floating support for six months and ensure they are linked to necessary support services afterwards. To provide assertive client-centred key-working, risk assessment, and support planning to a caseload of approximately ten clients to support them in maintaining their current accommodation and linking them to services and meaningful activities that help them improve their physical and mental well-being and life skills.

Responsibility

Provide a high-quality, customer-focused service, treating clients courteously and respectfully. Meet all standards and targets relating to communication with residents. Assist and motivate residents to reach set goals and progress along an identified clear pathway for housing. Provide client-centred support plans and intensive assertive key-working support using reflective practice techniques.

Qualification – Essential

* GCSE Grade A-C (or equivalent) in English Language and Mathematics.

* Degree level or equivalent experience in a similar role.

* Advanced –Microsoft Office, Word, Excel, Outlook, social media and CRM.

Essential - Skills, Abilities & Experience

* Knowledge of the impact of Adverse Child Experiences and the principles of Trauma Informed Care when dealing with homeless people who have complex needs.

* Knowledge of the Care Act and Housing legislation and the available support for single homeless adults.

* Knowledge of welfare benefits and other support available to supported accommodation residents.

* Health and Safety issues about a supported housing setting.

* Experience working with homeless people with complex support needs.

* Experience in recovery-focused and harm-minimisation service provision for clients with multiple and complex needs.

* Experience in risk assessment, critical working and support planning.

* Experience of case coordination in a multi-agency setting.

* Ability to support and motivate residents of supported accommodation.

* Ability to provide holistic client-centred assessments for residents with complex support needs.

* Ability to communicate well with clients and colleagues.

* Ability to plan and organise own workload.

* Ability to record written/financial information accurately.

* Ability to prepare simple reports and correspondence.

Essential Compliance Requirements

* Five Years of Reference.

* Enhanced DBS Checks are required for the role.

* A pre-engagement screening is mandatory for this role.
