As a Home Support Worker, you'll provide housing-related support to vulnerable and older people, helping them stay happy, safe and independent in their own homes. You'll build trusted relationships, identify individual needs and support customers to achieve meaningful outcomes through practical guidance and signposting to local services.
What you’ll do:
* Deliver person-centred, low-level housing-related support to customers in their own homes.
* Carry out holistic assessments and identify customer needs and risks.
* Co-produce support plans, action plans and exit plans using the Outcome Star methodology.
* Support customers to achieve their goals and maintain their independence.
* Signpost individuals to community, voluntary and statutory services that can provide additional support.
* Maintain accurate records using a case management system and track customer progress.
* Complete case studies and help demonstrate the positive impact of the service
Who you are:
* A compassionate, empathetic and non-judgemental individual who genuinely wants to help others.
* Has previous experience of working in Adult Social Care (desirable)
* An excellent communicator who can build honest and trusting relationships.
* Organised, resilient and able to manage your workload effectively.
* Confident working independently and using your initiative to solve problems.
* Comfortable using ICT and Microsoft packages, digital systems and maintaining accurate records.
* Experienced in supporting vulnerable people, or passionate about developing a career in support services.
