Job Summary
We are seeking a compassionate and dedicated Domiciliary Support Worker to provide specialised assistance young male service user with complex needs. Having experience with working with service users who have Duchenne's muscular dystrophy is advantageous.

The position is Monday to Sunday 9am to 10.30am, 4pm to 5.30pm and 9.30pm to 11pm each day and you will be working with a second carer during these times.

The successful candidate will work closely with the service user and his family to deliver personalised care plans, ensuring the safety, comfort, and well-being of the service user in various settings. This role offers an opportunity to make a meaningful difference in the a wonderful young man who requires ongoing support, fostering independence and development. The service user will also have NIV overnight and requires support with all activities of daily living.

The service user is a very enthusiastic cook and really enjoys cooking and going out other community activities.

He also has a cat and dog

Responsibilities

* Deliver personalised care in accordance with individual care plans, focusing on the specific needs of each child

* Assist with daily activities such as personal hygiene, feeding, mobility, and medication administration where applicable

* Support during outings and activities, ensuring their safety and engagement

* Maintain accurate records of care provided and any changes in condition

* Communicate effectively with families, healthcare teams, and other professionals to coordinate care and share updates

* Drive to appointments, activities, or cook classes as required, ensuring safe transportation

* Moving and Handling

* Adhere to all health and safety protocols, including infection control measures

Experience

* Previous experience working with complex care, particularly those with complex or Duchenne's muscular dystrophy

* Experience in developing or following care plans tailored to individual requirements

* Strong communication skills to liaise effectively with families, colleagues, and external professionals

* IT proficiency for maintaining accurate documentation and reporting
