Lead Support Worker

County Durham & Darlington

Full time (37.5 hpw) - Temporary until March 2025

Pay £12.61 per hour (salary £24,664 per annum) with great benefits including Health Cash Plan

As our Lead Support Worker, you’ll be at the heart of supporting our customers who have been referred to us by the Durham & Darlington NHS crisis team to support their patients with a range of support needs including housing, money and sign posting allowing the crisis team to focus on focus on the patient’s mental health. You will fully integrate into the NHS Crisis team. This is a fast-paced service reactive to the needs of the crisis team aiming to reduce the crisis period patients are in and reducing the need for hospital admission. 

Typical day as a Lead Support Worker (known by us as a Support Coordinator)

Creating support plans with your customers

Customer group - mental health

Helping customers with support issues placing them in crisis including homelessness, benefit issues, debt and sign posting to relevant services outside of Home Group

Complete risk assessments, support planning, goal setting and regular reviews.

You bring

Passion to support our customers to live their best life, working collaboratively with an eye for detail.

Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.

Experience of coordinating and assessing customer referrals.

The ability to work on your own initiative, remain calm under pressure and have a resilient approach

A caring and empathetic attitude

Job details 

Where we’re flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.

Work pattern- as we need to attend meetings each morning at 8am this will be a mixture of 8am-4pm or 9am-5pm. We don’t work weekends.

A full UK driving license is essential for this role and to have access to a vehicle which is insured for business purpose.

Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.

You’ll need an Enhanced DBS check done and we pay for that.
