Description:

Marks Consulting Partners are currently looking for a Specialist Support Worker to work with one of our Housing Association clients.

What the Job Will be doing

* Managing a caseload of customers with complex and multiple support needs, delivering tailored, person-centred support.

* Carrying out strengths-based assessments, risk assessments and developing outcome-focused support plans.

* Supporting customers to sustain their tenancies and prevent homelessness through early intervention and practical support.

* Working proactively with customers experiencing homelessness, rough sleeping, poor mental health, substance misuse and social exclusion.

* Providing advice and support around welfare benefits, budgeting, financial wellbeing and income maximisation.

* Supporting customers to access healthcare, mental health services, substance misuse services, education, training and employment opportunities.

* Conducting home visits, outreach work and community-based support to engage customers who may be difficult to reach.

* Working collaboratively with local authorities, healthcare providers, probation services, housing providers and voluntary sector organisations.

* Identifying safeguarding concerns, managing risk and making appropriate referrals.

* Advocating on behalf of customers and promoting independence, resilience and long-term positive outcomes.

* Maintaining accurate case records, support plans and performance information using case management systems.

* Participating in evening and weekend rotas where required to meet service needs.

What You Will Need

* Experience supporting vulnerable adults with complex needs within housing, homelessness, supported housing or health and social care.

* Experience managing a caseload and developing personalised support plans.

* Good understanding of homelessness, tenancy sustainment and housing-related support.

* Knowledge of safeguarding, risk assessment and trauma-informed practice.

* Experience working with customers experiencing mental ill health, substance misuse, homelessness or social exclusion.

* Strong partnership working skills with statutory and voluntary sector organisations.

* Excellent communication, advocacy and relationship-building skills.

* Ability to manage competing priorities while maintaining accurate case records.

* A compassionate, resilient and solution-focused approach to supporting vulnerable people.

* Relevant qualification in Housing, Health & Social Care, Community Work or a related discipline (desirable)
