Rate of pay: £30 - £33 p/h umbrella days
Must holder a Valid Full UK Drivers License
Position Overview: We are seeking a skilled and compassionate Social Worker to provide specialized care to individuals within the Review Team . The ideal candidate will have a strong background as a Social Worker and a commitment to promoting recovery and rehabilitation for adults with physical disabilities and their families and carers
Key Responsibilities:
The post holder will be part of the Review Team (RT) in Adult Social Care working with adults with physical disabilities and their families and carers in Islington. The post holder will be accountable to Adult Social Care Services for ensuring that adults are reviewed under the Care Act 2014 Assessment to check they continue to be eligible for services and to provide services only to people who meet these criteria. The post holder will be involved in project work which will include three projects. This entails reviewing the top 20% of high packages of care, Homecare mobilisation project and assessing residents that attend in house day centre. The post holder will be expected to carry out a holistic review’s and explore if heath funding can be applied.
Requirements and qualifications required for this role:
Current and valid registration with the SWE.
Proven experience as a social worker.
Strong communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Commitment to providing high-quality patient care.
