We are currently seeking a dedicated Social Worker with experience in community mental health care, ready for a new opportunity to make a difference in Slough.

A Day in the Life of a Community Social Worker
As a social worker specialising in community mental health care, your role is pivotal in providing support and care to patients:
Conducting home visits to assess and ensure the well-being of individuals within the community.
Administering medications according to prescribed schedules to address patients' needs.
Engaging in empathetic conversations with patients to comprehend their conditions, facilitating effective assistance and monitoring their progress or setbacks.
Performing risk assessments when necessary to ensure patient safety and promoting a secure environment during treatment.
Requirements and Qualifications:
Active registration with the National Association of Social Workers or equivalent accreditation.
Demonstrated experience in community mental health care.
Strong communication skills and ability to thrive in high-pressure environments.
Ability to work autonomously and collaboratively within a multidisciplinary team.
Commitment to delivering compassionate and high-quality care to patients.
