Social Worker – Children with Disabilities Team - Herefordshire Council
Location: Hereford
Pay/Salary: £41,363 - £45,777 + £10,000 Welcome Payment
Office Working: 2-3 Days per week
Contract: Permanent
We are currently working with Herefordshire Council to find an experienced Social Worker for their Children with Disabilities Team within Children’s Services. With a stable management team and a mostly permanent workforce, Herefordshire are looking for someone with solid experience to be able to pick up a caseload and support their services.
Duties
Undertake statutory visits and completing S17 Assessments and S47s, parenting and sibling assessments, evaluating needs of the child and family.
Developing care plans for children and families, including provisions such as respite and short breaks.
Working closely with partner agencies, such as Health, Education, and Police.
Chair CIN, Core Group and professionals’ meetings, as part of the multi-disciplinary team.
