Social Worker – Home First (Hospital Team)

£38.43 Umbrella (£30.50 PAYE) per hour
Monday to Friday | Full-Time | On-Site
Service Care Solutions are currently recruiting for an experienced Social Worker to join our client within Swansea Local Authority, working as part of a fast-paced and dedicated Home First Hospital Team.
This is a full-time position (Monday to Friday) with an initial 2–3 month ongoing contract, with a strong likelihood of extension.
The Role: You will support individuals to safely transition from hospital back into the community, promoting independence and reducing unnecessary hospital stays.
Key responsibilities include:
Completing Care Act assessments and support planning
Supporting safe and timely hospital discharges
Working within a multi-disciplinary team (MDT)
Conducting Mental Capacity Assessments and Best Interest decisions
Liaising with families, carers, and external agencies
Managing a complex and fast-paced caseload
Responding to safeguarding concernsRequirements:
Must be registered with Social Care Wales
Experience within a hospital discharge, D2A or Home First setting
Strong knowledge of Care Act, MCA, and safeguarding legislation
Ability to work effectively in a high-pressure environmentContract Details:
£38.43 per hour Umbrella / £30.50 per hour PAYE (exclusive)
Full-time – Monday to Friday
Initial 2–3 month contract
