Social Worker
An exciting opportunity has arisen within the Adults Learning Disability Team. The successful applicant(s) will meet the requirements of the Professional Capabilities Framework (PCF) and Knowledge and Skills Statement (KSS Adults) applicable to the role of either Social Worker or Experienced Social Worker.
The function of the social work role is to provide a professional social work service to adults with a learning disability, their families and carers who are ordinary residents of Hammersmith and Fulham in line with the requirements of the Care Act 2014 and other appropriate legislation. This involves engaging with individuals, carers, and families to assess and intervene and using professional judgement employ a range of interventions promoting choice, control and independence.
The post holder will uphold the values and behaviours of Hammersmith and Fulham Council and possess developed specialist knowledge of social work practice in adults with a learning disability and be registered with Social Work England. The roles consist of working closely with other organisations to ensure the safeguarding of our customers.
The post holder will act in accordance with the principles of personalisation, ensuring that care and support are person centred and as far as possible put the people with whom they work in control of their lives. The postholder’s practice will be responsive, inclusive and community based. They will work within the legislative frameworks and agreed Local Authority policies and procedures. This will include taking appropriate safeguarding action as and when required. They will ensure that as part of the assessment in accordance with s.9 and s13 or the Care Act 2014, NHS Continuing Healthcare is considered for all individuals, and where it is considered an individual has health care needs, a referral to the relevant health authority is made in a timely way.
Tasks will include carrying out assessments of need, planning of services and reviewing outcomes with the individual, their personal networks and support providers. The role holder will actively promote wellbeing and independence, to enable individuals and carers to retain or regain their skills and prevent, delay or reduce deterioration. They will engage effectively with families to facilitate contingency planning to anticipate complexity and changing circumstances.
The post holder will deliver services that are co-produced with residents by actively encouraging involvement and ownership of the support plan but also take a leadership role where appropriate. They will demonstrate confident and effective judgement about risk and accountability in decision making. They will work inclusively, with a diverse range of stakeholders and promote equality of opportunity.
As a social work professional, the post holder will bring significant expertise to complex cases in the Adult Learning Disability service.
