Assessment and Recovery Planning.
Conduct specialist addiction assessments to identify the physical, psychological, and social impacts of substance use.
Assist individuals in developing personalized recovery plans, setting realistic goals for both abstinence and harm reduction.
Evaluate new referrals and signpost individuals to appropriate partner services (e.g., detox, residential rehab, or mental health teams).
Provide immediate support for individuals in crisis, including relapse prevention and harm reduction advice (e.g., Naloxone training).
Help service users navigate practical barriers such as housing, welfare benefits, and attending medical appointments.
Act as the lead professional for complex cases, ensuring that different agencies (GP, Criminal Justice, Housing) are communicating effectively.
Identify and manage risks to children or vulnerable adults within the household, participating in Child Protection and Adult Support and Protection (ASP) case conferences.
Prepare high-quality social work reports for Courts, Children's Hearings, or Adult Protection inquiries.
Execute duties under the Social Work (Scotland) Act and Mental Health Act where relevant to substance misuse.
Work alongside Nurses, Doctors, and Occupational Therapists within the Midlothian Health and Social Care Partnership.
Foster relationships with local recovery communities, such as the Horizons Recovery Café and SMART Recovery groups.Requirements:
