Senior Individual Assessment and Commissioning Nurse

Location: Remote - induction in Warrington or Liverpool

Contract Type: Temporary - ongoing contract

Hours: Full-time or Part-time

Shift Pattern: M-F 9-5

Job Summary

Cheshire and Merseyside Integrated Care Board is seeking an experienced and highly motivated Senior Individual Assessment & Commissioning Nurse (IACN) to join their Individual Assessment and Commissioning Team.

This is an exciting opportunity for a registered nurse with experience in Continuing Healthcare (CHC), case management, commissioning, or complex care assessment to play a key role in ensuring individuals receive high-quality, person-centred care tailored to their needs.

You will manage a diverse caseload, undertake assessments and reviews, commission and monitor care packages, and work collaboratively with health and social care partners to deliver safe, effective, and cost-efficient services

You are a Registered Nurse with experience in Continuing Healthcare, mental health or community care.

You will bring:

Key Responsibilities

Undertake assessments and reviews in line with the National Framework for NHS Continuing Healthcare (CHC), NHS Funded Nursing Care and relevant legislation.
Manage a caseload of individuals eligible for NHS-funded care, ensuring timely and person-centred case management.
Commission and review packages of care to ensure quality, safety, and achievement of identified outcomes.
Support fast-track referrals, urgent care arrangements, and end-of-life care pathways.
Contribute to Section 117 aftercare assessments and package reviews where applicable.
Work closely with service users, families, providers, local authorities, and multi-disciplinary teams.
Provide specialist advice and guidance relating to assessment, commissioning, and continuing healthcare processes.
Support quality improvement initiatives, audits, training, and service development activities.Essential Skills and Experience

Current NMC registration
Experience within Continuing Healthcare, mental health or community nursing
Knowledge of the CHC National Framework, Mental Health Act and safeguarding
Experience managing caseloads and coordinating care
Strong communication and organisational skills
Ability to work independently and manage competing priorities
IT skills, including Microsoft Office and accurate data entryDesirable Skills and Experience

Experience of leadership or line management.
Strong understanding of personalised care and Personal Health Budgets (PHBs).
Knowledge of Local Authority functions and partnership working arrangementsBenefits
