Good Afternoon,

I am currently representing Southwark Council, who are offering an initial temporary contract for a period of 3-6 months with a view to be extended or offered a permanent position for the right candidate at a rate which is negotiable dependent upon experience

We are looking for a SEND Progression Mentor this role will be based at: SE172QB

The right candidate will:

The role is to work in the Gateway team. You will be working with persons with Learning Disabilities, Autism and Sensory needs, Physical Disabilities, or a range of these.

* To provide person-centred support for people with complex needs, by developing communication skills and participation/engagement in tailored activities to their maximise opportunities and increase independence.

* To act as keyworker in identifying and facilitating person-centred, meaningful EHCP outcomes with the person, and their wider support networks.

* To plan appropriate interventions to maximise social inclusion, wellbeing and promote independence and resilience.

* To connect people with their local network, Community Voluntary Sector and their wider community to promote independence and well-being.

* To identify what assets, support networks and local resources are available and accessible to persons.

* To provide advice, information and connect individuals and their support networks to local community services. To have the opportunity to have choice and control over the support they access.
