Are you a Registered Learning Disabilities Nurse with experience completing Continuing Healthcare assessments?

This is an opportunity to use your clinical expertise across complex Learning Disability CHC cases, reviewing existing care arrangements and helping to redesign person-centred packages of care.

The role
You will undertake complex CHC assessments for individuals with learning disabilities who receive Continuing Healthcare funding.

You will review clinical records, assess ongoing support needs and make clear, evidence-based recommendations about the care and funding required. Working with commissioners, care providers and Local Authority representatives, you will help ensure care packages meet each person’s assessed clinical and social care needs.

Your key responsibilities will include:
• Completing complex Learning Disability CHC assessments and reviewing clinical evidence
• Assessing ongoing health, social care and support needs
• Reviewing and redesigning complex packages of care
• Producing clear clinical reports and evidence-based recommendations
• Supporting individuals and families throughout the assessment process
• Working closely with commissioners, Local Authorities, care providers and other professionals

What we’re looking for
You must be a Registered Learning Disabilities Nurse with current RNLD registration and experience completing Learning Disability CHC assessments.
You will also need experience reviewing or redesigning complex packages of care, strong clinical decision-making skills and the ability to produce clear, evidence-based reports.

You should be comfortable managing complex assessments involving multiple stakeholders and working independently in a remote role. Experience completing assessments remotely would be advantageous.
Salary and working arrangements

The advertised salary is £39,959 to £48,117. Candidates with particularly strong and relevant experience who are seeking a higher salary are still encouraged to apply, as salary expectations may be considered individually.
This is a permanent, full-time position working 37.5 hours per week remotely.
