Description:
Belmont Recruitment are currently looking to speak to Registered Mental Health Professionals for a role within a small High Intensity Users Service. This service works with people who have repeat inpatient admissions or frequent contacts with Urgent Care/A&E/WMAS.
This role is a 6-12 Month Rolling Contract.
Within this role you will:
work across various sites and community settings, including service users’ homes, providing comprehensive assessment, liaison, collaboration and support to service users and the individuals/teams who work with them.
Be proficient in assisting the service user to co-produce a care plan which facilitates admission avoidance and reduction in urgent care contacts.
Act as a resource of mental health expertise by working closely with in and out service user colleagues, to ensure a psychologically informed approach to support requested and required.
Working Hours for this role are 3 days per week across a Monday- Friday Rota 9-5.
Rate for this role is £25PH.
The applicant must be a Qualified Registered Mental Health Nurse.
