Job Description: Overview of Main Responsibilities of Team Leader Post:

To provide day-to-day management support of the multi-disciplinary team in delivering community care to service users.
To work closely with the Team Manager and other senior workers to develop the Community Mental Health Team.
To establish strong working relationship with the in-patient Service, Hillingdon HTT, Hillingdon Psychiatric Liaison Team and CNWL
To monitor workload and ensure fair distribution according to the various team roles
To attend monthly interface meetings and contribute to improving local care pathways.
To regularly undertake delegated managerial responsibilities and duties, and to deputise for the Team Manager in his/her absence
To undertake complaints and serious incident investigations in accordance with the Trust's Policies.
To offer regular structured supervision to staff within the team, as well as providing informal support
To undertake regular caseload scrutiny for categories such as discharge, step down or step up care
To participate with the Team Manager in staff appraisals.
To maintain clear, accurate and up-to-date recording of client work.
To work according to the principles of equal opportunities in all dealings with patients, staff, other professionals and the general public
To undertake regular audit of client case notes to ensure high standards of delivery of care are achieved and maintained

What you'll need as a Registered CPN:

NMC registration and educated to degree level
Experience in assessment
6 months of paid patient facing UK nursing experience from within the past 3 years
Up-to-date clinical best practice knowledge
The right to work in the UK
