We are looking to recruit Social Worker within the Review Team. Our client has a well-structure and organised team that are looking to grow their staffing levels. The role offers flexible working hours and has a supportive team culture with regular supervision on cases. Wandsworth is embedding a systemic approach and is currently undergoing several changes focussed on reducing caseloads and improving outcomes for families. This is an exciting opportunity to join a fast-paced and supportive team within Wandsworth Council.
Main duties include:
Carrying out reviews with people residing in the community and also in nursing/residential settings
Reassessments
Working within the FACs criteria and Personalisation agenda
Managing a caseload of Adults to be reviewed for care
Working Multi-disciplinary team
Attending meetings and Liaising with other professionals
