Are you a passionate and dedicated Social Worker looking to make a meaningful difference in the lives of adults in need? Join our dynamic team in North London as a Qualified Social Worker in the Adults Hospital Discharge Team. We are seeking an individual with experience and compassion to support vulnerable adults transitioning from hospital to their homes or alternative care settings.
Key Responsibilities:
* Conduct assessments of individuals' needs upon discharge from hospital, ensuring a person-centered approach.
* Develop and implement care plans in collaboration with service users, families, and multidisciplinary teams.
* Coordinate services and resources to facilitate smooth transitions for individuals leaving hospital care.
* Provide advocacy and support to ensure service users' rights and preferences are respected.
* Collaborate with community partners, health professionals, and local authorities to optimize care delivery.
* Maintain accurate and up-to-date records in compliance with regulatory standards and organizational policies.
Requirements:
* Qualified Social Worker with a recognized social work qualification (e.g., DipSW, CQSW, CSS, or equivalent).
* Registered with Social Work England.
* Previous experience working with adults in a social care setting, preferably in hospital discharge or community care.
* Strong assessment and case management skills.
* Excellent communication and interpersonal abilities.
* Ability to work effectively within a multidisciplinary team and independently.
* Knowledge of relevant legislation, policies, and procedures related to adult social care.
