About the role: As a Social Worker in the Children with Disabilities Team, you'll play a vital role in improving the lives of children and young people with disabilities. Your responsibilities will include:

Assessing the needs of children and young people with disabilities, determining if they require specialist intervention or can access mainstream activities.
Evaluating the needs of children with severe and profound disabilities, specifically focusing on short breaks support for them and their families.
Creating care plans for children in various situations, ensuring their needs are met by all involved in their care.
Conducting home, school, and community visits, with a focus on direct work with children and young people using suitable communication methods.
Collaborating with health, education, and other professionals to ensure the comprehensive needs of children and young people are met.

About you:

Hold a Social Work qualification and have completed your Assessed and Supported Year in Employment (ASYE). ESSENTIAL
Be registered with Social Work England. ESSENTIAL
Ideally, possess experience working directly with children and young people with significant disabilities. However, experience in safeguarding is also valuable.
Be willing to work across two London Boroughs (Richmond and Kingston) while being based in Chessington.
Be comfortable working in an office environment, with a hybrid working model available.
Be a team player, actively participating in the team duty system, supporting colleagues and other professionals, and maintaining a positive attitude towards your work.
