Qualified Social Worker - Community Mental Health Team

Post Pay Level: 7 - £38,223 to £43,421 plus £1039 Local Weighting Allowance per annum

Location: Slough Community

Reports to: Team Manager

DBS Requirement: Enhanced

Main Purpose of Job:

As a Social Worker within the Slough Community Mental Health Team (CMHT), you will work as a vital member of a specialist mental health team, focusing on assessment, care planning, and service purchasing. Operating within the framework of relevant legislation, your role will involve utilizing social work experience in a multi-disciplinary team.

Main Accountabilities:

Manage a caseload and provide advice, therapeutic interventions, and advocacy according to professional expertise for clients and carers.
Work with service users who are difficult to engage and have chaotic lifestyles, adapting to unpredictable and challenging situations.
Engage in the CPA process to provide enhanced care, including evidence-based psychosocial interventions, coordination of services, family/carers involvement, and needs-based reviews.
Negotiate service delivery with providers, ensuring compliance with service level agreements and statutory responsibilities.
Monitor progress, amend objectives, and ensure cost-effectiveness and compliance with agreed CPA and Care management Policies and Procedures.
Participate in activities benefiting from professional skills, such as working parties, policy formulation, group work, and community development work.
Inform the Services Manager of specific needs and challenges in service provision.
Undertake investigations and prepare reports for panels, case conferences, and courts.
Contribute to standards of practice, training, and development activities, including the supervision of students and care co-coordinators.
Collaborate with the CRHTT to facilitate admission and discharge to and from inpatient units.
Engage in training activities to support good practice and the development of the whole service.

Experience/ Qualifications:

Qualified Social Worker with a recognized professional qualification (e.g., BA Social Work, MA Social Work, DipSW): Essential
Current registration with Social Work England: Essential
Experience of working with families/individuals in a planned way: Essential
Experience of working with Mental Health and Child protection: Essential
Experience of balancing complex workloads and setting priorities: Desirable

Skills and Abilities:

Good verbal and written skills: Essential
Ability to work within a changing organization: Essential
Ability to work with staff and managers at all levels and across Health and Social Service disciplines: Essential
Ability to provide supervision if required: Essential
Ability to work and contribute to a multi-disciplinary team with the ability to motivate self and others: Essential
Ability to use IT packages, including Word and Excel: Essential
Ability to converse at ease with customers and provide advice in accurate spoken English: Essential
Knowledge and Understanding:
Understanding of working with individuals and adults who have drug/alcohol problems: Desirable
Knowledge and understanding of Child and Community Care Legislation, including the Mental Health Act 1983: Essential
Understanding of what constitutes a high-quality service: Essential
Understanding and commitment to equality and diversity: Essential

Qualifications:

Recognized social work qualification CQSW CSS, Dip SW: Essential
Practice Teacher award: Desirable
Registration with the General Social Care Council to comply with National Requirements: Essential
Evidence of professional and personal development: Essential

Other Requirements:

Full valid driving license and daily use of a car: Desirable
