Randstad are recruiting for a Qualified Social Worker to join Liverpool City Council's Discharges from Community Intermediate Care Centre . This is a full time role, offering an amazing pay rate of £33 per hour.

Discharges from Community Intermediate Care Centre (CICC) based at Clatterbridge Hospital.
Working in an MDT setting to improve adult patients and their carers experiences, avoid hospital admission and reduce long length of stays.
Assessing people who have been placed on a discharge to assess (D2A) pathway from hospital into nursing homes.
Work with adults and their carers to provide access to information, advice and services including support and access to information to enable self-care and self-management.
Assess long term care needs via the 3 conversation model and develop support plans to enable people to return to their own homes where appropriate or another community setting.
Where a person may not have capacity for a decision about their support plan/discharge arrangements, apply the Mental Capacity Act 2005 informed by the MCA Code of Practice and relevant case law.Requirements:

Social Work Degree or equivalent
Social Work England registration
Enhanced DBSExperience:
