About the Role:
Dorset Council is seeking a qualified Social Worker to join their Home First Team, dedicated to facilitating efficient and effective hospital discharges. This is an excellent opportunity for a practitioner with a passion for strength-based practice and experience in complex case management including work related to the court of protection.
Day-to-Day Duties:
Support hospital discharges by coordinating with healthcare providers and community resources.
Conduct assessments and develop care plans that promote independence and well-being.
Work collaboratively with individuals, their families, and multidisciplinary teams to ensure smooth transitions from hospital to home.
Apply a strengths-based approach to support individuals in achieving their personal goals.
Navigate complex cases, including those involving the Court of Protection, ensuring legal and ethical standards are met.
Travel within the community to provide support and conduct assessments as needed.Requirements and Qualifications:
Qualified Social Worker with current registration.
Proven experience in the hospital discharge process.
Strong understanding and application of strengths-based practice.
Experience or knowledge of working with complex cases and the Court of Protection.
Excellent communication, assessment, and organizational skills.
Ability to work independently and as part of a team.Expected hours: 37.00 per week
