???? Role duties

As an Independent Prescribing Pharmacist at Suvera, you’ll provide specialist pharmaceutical support to patients working within your clinical competencies. Supporting our NHS partners, you’ll be patient facing and focused on proactively managing patients living with long-term conditions.

From repeat prescription management to structured medication reviews, it’s a varied workload. You’ll also be involved in quality improvement and audit and managing some aspects of the Quality and Outcomes Framework (QOF). You will receive support to help develop the role.

Clinical responsibilities

Collecting and recording patient health data in a holistic manner

Interpretation, risk stratification and management of data in line with standard operating procedures and NICE guidelines

Providing evidence-based lifestyle advice

Prescribing or changing patient medication as appropriate

Providing clinical leadership to members of the team
Non-clinical responsibilities 

Maintaining patient safety and dignity at all times

Enrolling patients onto the Suvera platform

Building and maintaining patient trust

Maintaining clinical notes on Suvera and GP partner systems

Proactive involvement in QOF

Liaising with members of the operations/product/technology teams

Participating in shared learning with Suvera and our GP partners

Taking a proactive role in audits relating to patient activity

Assisting in development of Suvera systems and services

Undertaking other administrative duties where reasonably required

???? Who you are

Applicants must have a minimum of 12 months experience as a clinical pharmacist within an NHS primary care setting. In addition, you will be:

A current member of a medical professional defence organisation

An independent prescriber and have completed a GPhC-accredited course/or equivalent

Currently registered with the GPC and act at all times within the boundaries of it’s code of professional conduct

An excellent verbal and written communicator

Confident communicating with patients via their preferred communication channel which could include email, messaging, telephone or video calls

IT literate, and able to work with a variety of software systems, including but not limited to Emis Web, SystmOne, Microsoft Office

Confident managing the following conditions: Hypertension, Type 2 Diabetes and Asthma
