We are seeking a dedicated and experienced Clinical Pharmacist to join our team at a busy GP surgery in Lewisham. The ideal candidate will play a key role in patient care, working closely with our GPs and healthcare team to provide expert medication management and support to our diverse patient population.

Key Responsibilities:
- Medication Management:
- Conduct comprehensive medication reviews for patients with chronic conditions.
- Manage and optimize the use of medicines for patients, including initiating, adjusting, and discontinuing medication as appropriate.
- Provide advice on the safe and effective use of medications.
- Patient Care:
- Work directly with patients to improve medication adherence and educate them about their treatment plans.
- Address and manage any medication-related queries or issues from patients and other healthcare professionals.
- Monitor patient outcomes and ensure medications are used safely and effectively.
- Clinical Support:
- Support GPs and other healthcare professionals in the management of patients with complex medication needs.
- Participate in multidisciplinary team meetings and case discussions.
- Provide expertise on prescribing practices, ensuring compliance with current guidelines and regulations.
- Quality Improvement:
- Contribute to the development and implementation of protocols and guidelines for medication management.
- Participate in audit and quality improvement projects to enhance patient care and safety.
- Stay updated with the latest developments in pharmacotherapy and primary care.
- Education and Training:
- Educate and train healthcare staff on pharmacological topics and best practices.
- Mentor and support pharmacy students and junior pharmacists.
Qualifications and Experience:
- Essential:
- Registered Pharmacist with the General Pharmaceutical Council (GPhC).
- Postgraduate qualification in clinical pharmacy or equivalent experience.
- Experience working in a primary care setting, preferably within a GP surgery or hospital.
- Strong clinical knowledge and the ability to apply this in a practical setting.
- Excellent communication and interpersonal skills.
- Desirable:
- Independent prescribing qualification.
- Experience with electronic prescribing systems.
- Knowledge of local healthcare services and patient pathways in Lewisham.
