Rehabilitation Case Manager

Full-time, Permanent (Part time would be considered)

Location: Midlands - Nottingham, Birmingham, Sheffield

Salary: £35,000.00 - £45,000 per year plus pension and bonus opportunity

A unique and exciting opportunity has arisen for a Case Manager to join our Clients highly experienced team. They are a well respected and long-established small case management business supporting adults and children who have sustained catastrophic injuries. They work together as a team, centred on promoting the rehabilitation of their clients.

As a Case Manager you will be required to assess and understand a person’s specific needs and use your clinical and professional skills to ensure maximum recovery. Full training and support will be provided.

To succeed in this role, you will need to have the ability to:

* Undertake assessments and provide client centred reports including recommendations for rehabilitation.

* Work alongside private and statutory sectors to co-ordinate robust rehabilitation plans.

* Ensure that case management and therapeutic input is regularly measured against client centred goals.

* Oversee the recruitment of support workers, providing training and ongoing supervision.

* Complete care plans and undertake regular risk assessments, ensuring that mitigating actions are in place.

* Hold a case load and manage your diary independently.

* Travel within the Midlands area

Requirements:

* A professional qualification such as RGN, RN, RMN, OT or Social Worker.

* At least 3 years post qualification experience.

* Experience within a case management role is preferable.

* You will require access to a vehicle and hold a valid UK driving licence.

* And most importantly excellent communication skills
