Newly Qualified Social Worker

Description:

Belmont Recruitment are currently looking for a Newly Qualified Social Worker to join a Local Authority’s Adult’s Social Care team in the Rochdale area. They are looking for someone to work 37 hours per week.

Support Planners will generally work with the less complex cases with guidance from Line managers and Health & Social Care colleagues within either the Adult Care Service or the Mental Health Service.

Key Duties:

* To ensure that adults who are eligible for care and support receive an assessment and any subsequent support necessary to meet their wellbeing needs

* Work with individuals and their family and friends to both assess needs and develop and review support plans in line with wellbeing outcomes

* Support an individual to take into account any preventive measures that will maintain their independence

* Work with individual to become less dependent of service support systems and develop in their place a self-support network of friends, family, etc

* Ensure individuals who are seeing job opportunities receive the necessary advice, support and guidance to move into employment

* Carry out outcome focussed assessments with carers to ensure needs are met

Requirements:

* Social Work England registration

* Experience of completing Care Act Assessments and commissioning support within a statutory social care setting

* Full UK Driving Licence and access to your own vehicle for work purposes

* Experience of working with mental health services – can be placements (desirable)

* Have an understanding of mental health pathways (desirable)
