About the Role
Are you passionate about supporting people in your community?
We are looking for a caring, motivated, and compassionate Band 5 Midwife Worker to join our team , You will play a key role in helping individuals maintain their independence, improve their wellbeing, and access the support they need within their own homes and communities.
What You'll Do
* Support service users with their health, wellbeing, and social care needs
* Carry out assessments and contribute to person-centred care plans
* Work closely with healthcare professionals, families, and partner agencies
* Promote independence and positive health outcomes
* Keep accurate and timely records
* Follow safeguarding, confidentiality, and professional standards
* Participate in team meetings, supervision, and training
What We're Looking For
Essential
* Minimum 6 months of NHS or community healthcare experience
* Experience working with vulnerable adults, families, children, or community services
* Good organisational and time management skills
* Understanding of safeguarding and confidentiality
* Full UK driving licence and access to a vehicle
