Are you a dedicated and experienced Social Worker with a passion for supporting looked after children and care leavers? Randstad Social Care is currently recruiting for a Looked After Through Care Team Social Worker to join a committed team in Redcar and Cleveland, focusing on providing high-quality support and intervention to children and young people transitioning from care to independence.

Role Overview:
We are seeking a compassionate and proactive Social Worker to join the Looked After Through Care Team in Redcar and Cleveland. In this role, you will work collaboratively with looked after children, care leavers, and partner agencies to support young people's transition from care to independence, ensuring they receive the necessary support and guidance to achieve positive outcomes.
Key Responsibilities:
Providing direct support and intervention to looked after children and care leavers, including assessment, care planning, advocacy, and practical support to promote independence and well-being.
Developing and implementing pathway plans for care leavers, focusing on education, employment, housing, and life skills development, in partnership with young people, families, and relevant professionals.
Facilitating access to appropriate services and resources for care leavers, including housing support, mental health services, financial assistance, and leisure activities.
Working collaboratively with partner agencies, including leaving care teams, education providers, housing services, and health professionals, to coordinate holistic support packages for care leavers.
Advocating for the rights and voice of looked after children and care leavers, ensuring their views and wishes are central to decision-making processes and service planning.Requirements:
Qualified Social Worker registered with Social Work England.
Experience working within a children's social care setting, with specific experience in looked after children and care leaver services.
Strong assessment, communication, and interpersonal skills, with the ability to engage effectively with children and young people from diverse backgrounds.
Knowledge of relevant legislation, policies, and procedures relating to looked after children, care leaving, and transitions to independence.
Commitment to promoting the rights, well-being, and aspirations of looked after children and care leavers, and ensuring their successful transition to independence.Benefits:
