Long Term Support Officer
£17.69 per hourly rate
London Borough of Tower Hamlets are looking for a Long-Term Support Officer.
You Will:
As part of a statutory social care team, provide a support officer function which includes the assessment and review of those with care and support needs and case management.
To ensure that prevention, strengths-based practice, integrated working, and person-centred intervention is central to all intervention.
To promote the safety and welfare of service users and carers enabling positive outcomes, working in line with legislation, policy, and practice guidance.
