Locum Band 5 Community Nurse – Keynsham, Bristol - £30 p/h
* £30 per hour
* Mon-Fri, 09:00-17:00

We are currently seeking a motivated and compassionate Locum Band 5 Community Nurse to join the District Nursing Keynsham Team in delivering high-quality healthcare services to patients in the community. The successful candidate will provide holistic nursing care, support individuals to manage their health conditions, and promote independence and well-being. This role involves working autonomously and collaboratively within multidisciplinary teams to assess, plan, implement, and evaluate care plans for patients of all ages across diverse settings.
Key Responsibilities:
* Conduct comprehensive assessments of patients' physical, psychological, social, and environmental needs within the community setting.
* Develop individualized care plans in collaboration with patients, their families, and other healthcare professionals, ensuring they are tailored to meet the specific needs and preferences of each patient.
* Provide skilled nursing care, including wound care, medication management, catheter care, and support with activities of daily living, in accordance with care plans and clinical guidelines.
* Administer medications and treatments safely and accurately, adhering to relevant policies and procedures.
* Monitor patients' health status, vital signs, and response to treatment, promptly identifying any changes or deterioration and initiating appropriate interventions.
* Educate patients and their families about their health conditions, treatment plans, and self-care strategies, empowering them to actively participate in managing their health and promoting healthy lifestyles.
* Coordinate and collaborate with other healthcare professionals, community agencies, and support services to facilitate seamless transitions of care and promote continuity of care for patients.
* Conduct home visits and assessments to support patients' transition from hospital to home, facilitate early discharge, and prevent unnecessary hospital admissions.
* Document all nursing interventions, assessments, and patient interactions accurately and comprehensively in electronic health records, ensuring confidentiality and compliance with data protection regulations.
* Participate in multidisciplinary team meetings, case conferences, and clinical supervision sessions to review patient progress, discuss complex cases, and contribute to care planning and decision-making processes.
* Adhere to professional standards, codes of conduct, and regulatory requirements governing nursing practice, including safeguarding vulnerable adults and children.
* Engage in continuous professional development activities to enhance clinical knowledge, skills, and competencies, keeping abreast of current evidence-based practices and emerging healthcare trends.
Qualifications and Experience:
* Registered Nurse with valid licensure and registration with the relevant regulatory body.
* Minimum of one year of post-qualification experience in a clinical nursing role, preferably in community or home care settings.
* Ability to work independently and collaboratively within multidisciplinary teams, demonstrating effective communication and interpersonal skills.
* Strong assessment, planning, implementation, and evaluation skills, with the ability to prioritize and manage caseloads effectively.
* Competence in clinical procedures, including wound care, medication administration, and catheter management.
* Proficiency in using electronic health record systems and other healthcare technology tools.
* Commitment to providing patient-centered care, promoting health equity, and advocating for the needs of diverse patient populations.
