Lead Practitioner - Homelessness Support

Our client is dedicated to providing essential assistance to individuals facing or recovering from homelessness, facilitating their journey towards securing and maintaining stable accommodation while nurturing independent living skills. As the Lead Practitioner within their team, you'll assume a multifaceted role incorporating both Visiting Housing Support and Preventative Support.

Key Duties and Responsibilities:

Visiting Housing Support:

Assist individuals in navigating housing applications.
Provide guidance and support throughout the housing procurement process, ensuring individuals have access to suitable accommodation options.
Collaborate with individuals and their support networks to address housing-related challenges and develop sustainable housing plans.
Offer practical assistance in settling into new accommodation, including coordinating moving logistics and addressing any immediate housing needs.
Preventative Support:

Employ a person-centered approach to engage with individuals and their support networks, including family, friends, healthcare professionals, advocacy services, and team members.
Provide proactive support aimed at preventing homelessness, such as budgeting assistance, financial management guidance, and access to benefits.
Foster independence by empowering individuals to manage day-to-day household responsibilities effectively.
Actively engage in ongoing training and professional development opportunities to enhance skills and knowledge relevant to the role.
