Hospital Social Worker – Adult Social Care
Location: Lambeth, London

Contract: 6-Month Maternity Cover
Pay Rate: £39.87 per hour Umbrella
Working Pattern: Hybrid – 3 days on site, 2 days from home
We are currently seeking an experienced Hospital Social Worker to join the Lambeth Hospital Social Work Team, supporting adults admitted to King's College Hospital and other hospitals both within and outside the borough.
This is an exciting opportunity for a skilled Social Worker with strong experience in adult social care, hospital discharge, safeguarding and complex assessments to join a fast-paced and supportive multidisciplinary environment.
Key Responsibilities

Carry out Care Act assessments and reviews for adults aged 18+ with a range of physical health needs, disabilities and vulnerabilities.
*
Complete strengths-based assessments and develop appropriate support plans.
*
Support safe and timely hospital discharges, working closely with patients, families and professionals.
*
Review existing packages of care and assess the need for ongoing support or placements.
*
Identify individuals who may benefit from reablement and make appropriate referrals to maximise independence.
*
Undertake safeguarding assessments and interventions in line with relevant procedures.
*
Work collaboratively with MDT colleagues, health professionals, community services and voluntary-sector organisations.
*
Coordinate with other teams and agencies across the borough to ensure appropriate support is in place.
*
Promote person-centred, strengths-based approaches and positive outcomes for residents.
*
Manage a varied caseload effectively within a demanding and changing hospital environment.
About You
We are looking for a qualified and experienced Social Worker who has:
*
Relevant experience in Adult Social Care and/or Hospital Social Work.
*
Experience working with adults with physical disabilities, health conditions and complex vulnerabilities.
*
A strong understanding of adult safeguarding procedures.
*
Experience of hospital discharge planning and working within a multidisciplinary environment.
*
Knowledge of the Care Act 2014 and strengths-based approaches.
*
Excellent assessment, care planning, risk management and decision-making skills.
*
Strong communication and partnership-working abilities.
*
The ability to work independently while contributing effectively to a wider team.
*
A flexible and resilient approach to working in a fast-paced environment.
Working Arrangement
This role operates on a hybrid basis, with a requirement to work 3 days per week on site in Lambeth and the option to work 2 days from home. This requirement is non-negotiable.
Pay: £39.87 per hour Umbrella
Contract: 6-month maternity cover
Location: Lambeth, London
Mandatory: Must have a UK Passport or a Full Right to Work in the UK
