The Role
* Personalized Care: Assist with hygiene, continence, oral care, and provide reassurance.
* Medication Support: Manage medications for adherence and safety.
* Household Support: Help with meal prep, laundry, and housekeeping.
* Transition Support: Aid in transitioning home from hospital stays.
* Community Access: Safely facilitate appointments and activities.
* Health Monitoring: Monitor conditions and skin integrity, report changes promptly.
The Candidate
* 1 year experience in Domiciliary/Home care (or related experience).
* Willing and eager to provide person-centred care.
* Good communication.
* A compassionate advocate for those under their care.
* Friendly and caring approach.
