Due to exciting new business growth, Citizens Advice Dudley & Wolverhampton is seeking a Health and Wellbeing Community Support Worker to join our dedicated team.

We provide comprehensive information, advice and guidance to individuals in need and we are expanding our team to meet the growing demand for our services and new ways of working.

Health and Wellbeing Community Support Worker- £23,151.48 – £24,491 FTE dependent on experience

p/w available (min 24hrs p/w would be considered)

Location: Dudley Offices / Other Venues / Home Working

Main Job Purpose

To identify opportunities to develop, coordinate and implement health and wellbeing initiatives and services for Dudley residents in line with service specifications and funders requirements relating to 3 key areas:
● 1) Health Ageing (for 65+ years residents);
● 2) Increasing social connectivity (and reducing loneliness and isolation);
● 3) Supporting the development of our self-management programme to improve quality of life for people with long term health conditions

Work alongside the Health and Wellbeing Co-ordinator and Service Delivery Manager/Head of Services to operationally deliver on activity related to the Health and Wellbeing strategic priorities and action plans
