Day Webster are currently recruiting a Grade 8 Occupational Therapist

What you need – Role requirements

An exciting opportunity has arisen for a Occupational Therapist to join STSS (short term support services) , STSS provides a single point of access to community intermediate care services, working with referring professionals to progress their initial assessment and provide the relevant services to the individual. Through collaborative working and holistic screening the team deploy and commission appropriate intermediate care services to ensure service users’ needs are met. The role will also involve holistic face to face assessments, reviews and support planning to support the transition onto appropriate services, please note that all assessments will be undertaken outside of the hospital setting. You will be using a strengths-based approach to ensure that people receive the right support at the right time that enables them to achieve their optimum level of independence and live their life in a way that they choose. You will be bringing the ethos and principles of a reabling approach to the team and supporting the team to work in an outcome focussed way. The ideal candidate will be someone who inspires colleagues through best practice and high quality, strength-based assessments and be able to manage risk to promote a 'home first' way of thinking with creative ideas to support people to remain in their own home environment. The ideal candidate will be articulate and have skills in building effective professional relationships with other internal and external professionals from social care, health and voluntary sectors. You will join a dedicated and motivated team of Health and Social Care professionals to support and facilitate discharges from hospital as well as collaborative working with our local Community Health & Social Care partners to prevent hospital admission. You will develop relationships and work collaboratively with external system partners and advocate the value and benefits to the service user of an integrated approach. You will have experience and skills in managing risk and promoting positive risk taking and be able to manage a varied caseload, including some complex cases. You will have a broad knowledge of equipment, moving and handling and compensatory techniques, including single handed care approaches to promote independence. You will need to be responsive, flexible and adaptable in a constantly changing environment as well as having a commitment to improve the lives of our service users through creative, evidence based practice.

HCPC Registration
