Are you an experienced Advanced Nurse Practitioner looking for a new challenge? We're recruiting for a dynamic and dedicated professional to join our Advanced Practice Team.
Role Summary:
* Provide primary nursing care services to patients of all ages.
* Offer health promotion and preventative care advice, including lifestyle modifications and disease prevention strategies.
* Work as part of the practice multidisciplinary team and supervise and provide guidance and support to allied health care professionals when needed
Role Responsibilities:
* Provide primary medical care to registered patients, including assessing, diagnosing, and treating a wide range of conditions, ailments, and injuries
* Adequately safety net all consultations to better ensure the safety of the patient.
* Manage acute and chronic illnesses, with a focus on preventive medicine and health promotion
* Show empathy, professionalism, and sensitivity whilst communicating with patients, ensuring a patient-centred approach that respects cultural and individual needs
* Maintain accurate and up-to-date patient records, ensuring confidentiality and adherence to GDPR and NMC guidelines.
* Collaborate effectively within a multidisciplinary team, including GPs, nurses, clinical pharmacists, healthcare assistants and administrative staff, to ensure seamless patient care
* Act as supervisor and mentor to other clinicians (including PAs) when required.
* Contribute to practice meetings, peer reviews, and continuous professional development (CPD) sessions to enhance knowledge and skills
* Stay updated on medical advancements, new guidelines, and regulatory changes relevant to primary care services.
* Follow local clinical governance policy and actively participate in quality improvement activities to continually improve the standard of delivered healthcare
* Actively participate in the surgery appraisal scheme and work towards achieving all aspects outlined on Personal Development Plan (PDP).
* Ensure that all statutory and mandatory training has been completed and remains in date.
* Adhere to registered professional codes and standards (NMC)
* Participate in practice audits, quality improvement initiatives, QOF and support implementation of updated protocols and guidelines when required
* Show empathy, professionalism, and sensitivity whilst communicating with patients, ensuring a patient-centered approach that respects cultural and individual needs
* Record and escalate any newly identified SEAs or complaints using the correct pathway.
Job Types: Full-time, Part-time
Pay: £45,000.00-£55,000.00 per year
