Experienced Adult Social Worker
Location: Wrexham
Rate: £44.23 per hour (Umbrella)
Contract: Full Time | Locum
We are currently recruiting for an Experienced Adult Social Worker to join a busy Adult Social Care team in Wrexham. This is an excellent opportunity for a skilled practitioner to manage a complex caseload while supporting vulnerable adults to achieve the best possible outcomes.
Working within statutory frameworks, you'll undertake assessments, manage safeguarding concerns, develop person-centred care plans, and work closely with health professionals and partner agencies to promote independence and wellbeing.
Key Responsibilities
* Complete strengths-based assessments and reviews.
* Develop and review care and support plans.
* Manage adult safeguarding enquiries and risk assessments.
* Undertake Mental Capacity Act assessments and support DoLS processes.
* Work collaboratively with health, housing and community partners.
* Manage a complex caseload and maintain accurate case records.
Requirements
* Qualified Social Worker with current Social Care registration.
* Experience working within Adult Social Care.
* Strong knowledge of relevant adult social care legislation, safeguarding and mental capacity.
* Excellent assessment, communication and case management skills.
* Ability to work independently and as part of a multi-disciplinary team.
