Team Support Healthcare are currently recruiting an experienced CPN Care Coordinator, in Cheshire & Chester areas.

Schedule:
* Day shift
* Monday to Friday

* Weekend availability
Experience:
* Experience with handling a caseload
* 6 months appropriate experience in the last 3 years
Licence/Certification:
* NMC (required)
* Valid DBS on the update service
* Right to work in the UK
Ability to Commute in Manchester (required)
