About the Role

Health Now Complex Care is looking for experienced and compassionate Complex Care Support Workers to join a specialist care package in Grimsby.

This is an opportunity to work as part of a dedicated care team supporting an individual with complex care needs within their own home.

We are looking for reliable, caring and confident support workers who are comfortable providing hands-on care and following detailed care plans, while promoting the individual's dignity, independence and quality of life.

Key Responsibilities

As a Complex Care Support Worker, your responsibilities will include:

Providing person-centred care and support within the individual's home.

Following the individual's care plan, risk assessments and clinical guidance.

Supporting with personal care and daily living activities.

Monitoring and reporting any changes in presentation, health or wellbeing.

Supporting with mobility and transfers where required.

Working alongside other members of the care team to provide consistent and safe care.

Maintaining accurate and timely care records.

Communicating effectively with family members, colleagues and healthcare professionals.

Promoting dignity, choice, independence and wellbeing at all times.

Following Health Now Complex Care's safeguarding, health and safety and infection prevention procedures.

What We're Looking For

We are looking for support workers who have experience working within complex care, supported living, residential care or community/home care environments.

You should be:

Compassionate and person-centred.

Reliable and punctual.

Confident working as part of a team.

Comfortable working long shifts.

Able to follow care plans and clinical instructions.

Confident communicating with colleagues and healthcare professionals.

Committed to providing high-quality care.

Able to demonstrate good understanding of safeguarding and professional boundaries.

Desirable Experience

Experience of working with individuals with complex physical health needs would be highly desirable.

Previous experience with:

PEG care

Tracheostomy care

Moving and handling

Personal care

Complex medication

Enteral feeding

Clinical monitoring

Community/home care

would be advantageous.

Full training and competency sign-off will be provided where required, subject to the needs of the package.

Shift Pattern

The package operates on 12-hour shifts, including:

Day shifts

Night shifts

Weekdays

Weekends

We are looking for staff who can offer regular availability and provide consistency within the care team.

Training & Compliance

All staff must complete Health Now Complex Care's recruitment and compliance process before commencing work.

Depending on the requirements of the package, staff may be required to complete relevant training and competency assessments before working independently.
