Calling all compassionate Community Nurses!

We're on the lookout for an enthusiastic and caring Band 5 Community Nurse to join our dynamic team in Barnet.

As a Community Nurse, you'll have the privilege of making a real difference in people's lives by providing high-quality care right in their homes.

Role: Community RGN - Band 5 

Type: Agency/Locum - NHS only

Start: ASAP

Duration: Ongoing

Your role will encompass:

Delivering comprehensive nursing care to patients in their homes, covering wound care, medication administration, and chronic disease management
Assessing patient needs and crafting personalized care plans tailored to their unique circumstances.
Empowering patients and their families with knowledge and strategies to manage their health effectively.
Collaborating seamlessly with other healthcare professionals, including GPs, social workers, and occupational therapists
Contributing to the development and implementation of innovative community nursing initiatives
Keeping patient records organized and up to date to ensure seamless care coordination
To excel in this role, you'll bring:

Registered Nurse (RN) status with valid Nursing and Midwifery Council (NMC) registration
Proven experience as a Community Nurse or in a relevant clinical setting
Exceptional communication and interpersonal skills to build trust and rapport with patients.
The versatility to work independently and collaboratively as part of a team.
Impeccable organizational and time management skills to navigate a busy schedule.
A valid driving license for convenient patient visits
