Description:

An exciting opportunity has become available for a caring and dedicated Community Reablement Assistant to join a specialist community reablement service supporting individuals in their own homes across Bournemouth.

This rewarding role focuses on improving lives and building independence by providing short-term, person-centred support to individuals who need assistance following illness, injury, hospital discharge or a change in circumstances. The aim is to help clients regain confidence, maintain their daily living skills and remain as independent as possible within their own homes and community.

As a Community Reablement Assistant, you will provide practical and emotional support to individuals, helping them achieve their personal goals and agreed outcomes. You will encourage independence, support daily routines and work alongside other professionals to ensure each person receives the right level of care and support.

What's on offer:

Hourly rates from £13.74 per hour (weekdays)
Weekend rates up to £16.83 per hour
Full-time position – 40 hours per week
Early and late shifts: 7:00am–2:30pm or 3:00pm–10:30pm
Rota covering weekdays and weekends

Responsibilities include:

Providing high-quality reablement support to individuals in their own homes
Supporting clients with everyday tasks to help them regain confidence and independence
Following individual goal plans and supporting clients to achieve agreed outcomes
Assisting with personal care, daily routines and practical tasks where required
Administering medication safely using MAR charts
Monitoring and reporting changes in clients' health, wellbeing or progress
Maintaining accurate care records and documentation
Supporting individuals to access community resources and maintain independence
Working alongside families, carers and other professionals involved in client support
Promoting dignity, choice, wellbeing and person-centred care at all times
What we're looking for:

NVQ Level 2 in Health & Social Care (or equivalent) – Essential
Previous experience working within social care
Experience administering medication using MAR charts
Ideally previous experience working within a community care or reablement setting
Full UK driving licence with access to a vehicle
Business use motor insurance is essential
Caring, reliable and professional approach
Good communication skills and the ability to work independently
Passion for supporting individuals to live as independently as possible
Please note:
This is a community-based role requiring regular travel to support clients in their own homes. The successful candidate must be flexible to work a rota of early and late shifts across weekdays and weekends.
