Description:

Job Summary: We are seeking a dedicated and compassionate District Nurse to join our community healthcare team. The successful candidate will provide high-quality nursing care to patients in their own homes, residential care settings, and community clinics. As a District Nurse, you will play a critical role in managing the health and wellbeing of patients with a wide range of medical conditions, promoting independence, and supporting end-of-life care.

Key Responsibilities:

* Deliver comprehensive nursing care to patients in the community, including wound management, medication administration, palliative care, and chronic disease management.

* Conduct holistic assessments of patients' needs, developing and implementing individualised care plans.

* Provide health education and advice to patients, families, and caregivers to promote self-care and prevent illness.

* Collaborate with other healthcare professionals, including GPs, social workers, and physiotherapists, to ensure integrated and coordinated care.

* Maintain accurate and timely patient records in accordance with NHS guidelines and confidentiality policies.

* Participate in case conferences and multidisciplinary team meetings to discuss patient care and progress.

* Support and supervise healthcare assistants and junior staff, ensuring high standards of care are maintained.

* Undertake regular training and professional development to keep up-to-date with best practices and advancements in community nursing.

* Respond to urgent and emergency situations, providing swift and appropriate care.

* Participate in the on-call rota, providing out-of-hours nursing support as required.

Qualifications and Experience:

* Registered Nurse (Adult) with current NMC registration.

* Minimum of 6 months of District / Community Nursing experience (preffered).

* Full UK driving licence and access to a vehicle for work purposes.

Skills and Attributes:

* Excellent clinical skills with a broad knowledge of community healthcare.

* Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and colleagues.

* Ability to work independently, manage time effectively, and prioritize workload.

* Empathy, compassion, and a patient-centered approach to care.

* Good IT skills, including proficiency in electronic patient records systems.

* Commitment to continuous professional development and lifelong learning
