This Role offers a block booking/shifts for up to November 2024, 5 Days per week; Hours 9am to 5pm

Responsibilities:
* Provide skilled nursing care to patients in their homes, delivering personalized care plans tailored to individual needs.
* Conduct assessments, monitor patient progress, and adjust care plans as necessary to ensure optimal outcomes.
* Collaborate with multidisciplinary teams, including physicians, therapists, and social workers, to coordinate comprehensive care for patients.
* Educate patients and their families on managing health conditions and promoting wellness within the community.
* Maintain accurate and up-to-date patient records in accordance with professional standards and regulatory requirements.
Qualifications and Requirements:
* Registered with the Nursing and Midwifery Council (NMC) as an RGN.
* Previous experience in community nursing or a related field is desirable but not essential.
* Excellent communication and interpersonal skills, with the ability to build rapport with patients and colleagues.
* Strong clinical skills and a commitment to delivering high-quality, patient-centered care.
* Valid driver's license and access to a vehicle for community visits (if applicable).
