HCC-Temps is seeking a compassionate and skilled Community Nurse to join our team. In this rewarding role, you will play a vital role in supporting the health and well-being of individuals and families in our community.

About Us:

At HCC-Temps, we supply all grades of staff to the healthcare sector ranging from healthcare assistants, support workers, registered nurses, chefs, kitchen assistants, and catering assistants in various hospitals and care home locations across the UK.

About the Role:

Provide a wide range of nursing care to patients in their homes, including assessments, wound care, medication management, and health education.

*

Develop and implement individualised care plans in collaboration with patients, families, and other healthcare professionals.

*

Promote health and well-being through preventative measures and early intervention strategies.

*

Liaise with other healthcare providers, social services, and community organizations to ensure continuity of care.

*

Maintain accurate and comprehensive patient documentation.

*

Contribute to the development and improvement of community nursing services.

Essential Skills and Qualifications:

* Registered Nurse with current UK NMC registration

* Evidence of other post-registration education and training

* Recent clinical nursing experience negotiating and liaising within a multidisciplinary team

* Working with people with complex health needs

* Experience in undertaking general assessments by utilising evidence-based care

* Post-registration experience in a community environment

* Knowledge of out-of-hospital care and the role of the Community Nurse

* Knowledge of PREP

* Ability to listen effectively and ability to give constructive feedback to clients and colleagues.

* Knowledge of how Equal Opportunities can be implemented in practice.

* Knowledge of reasonable response to service user needs and prioritising and dealing with those needs with a required urgency

* Awareness of the management and use of data in the NHS setting

* Knowledge of risks involved in working in the community

* Knowledge of evidence-based practice

* Knowledge of management of long-term conditions e.g. Diabetes and an understanding of the concept of self-management.

* Knowledge of recent government legislation and changes in community Care delivery

* Understanding of resource management

* Knowledge of the audit process

* Ability to work in a team be a proactive member and able to engage in the change process in the team.

* Ability to organise and participate in weekly work allocation and be able to prioritise and delegate effectively

* Ability to communicate well verbally and in writing such as computerised documentation and data entry

* Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)

NB: Ideal candidates should be able to get to these locations: Dartford, Gravesham, Swanley and Swale in North Kent.
