Trident Housing, which provides landlord services for the Group in respect of housing management and maintenance

These services include supporting people within registered care, domiciliary care, people with learning difficulties, those with mental health needs, young people at risk, older people, BME groups, people at risk of being homeless, those seeking employability, education and training opportunities and those fleeing domestic abuse.

About the role

Due to continual growth within our Shropshire Mental Health services, we are currently looking to recruit a Community Support Worker. The role will be based in the community where we currently support customers in Shrewsbury, Oswestry, Market Drayton, and surrounding areas of Shropshire. Our customers are supported within their own homes and our supported tenancy services.  Our aim is to provide a person-centred approach for everyone.

Demonstrating a commitment to safeguarding in line with policy and procedures you will support customers with life skills and empower them to build successful relationships within their community as they develop skills to better manage their health and well-being and increase levels of independence as they grow in confidence and manage their tenancy.

All our customers are respected with a person-centred approach and the support we give is customised to how they wish to spend their time with you. This could include going to sporting events and arenas, shopping, doctor appointments, voluntary work, social groups and more.

The role is also covered by some milage costings towards your Journeys between visits.

About You

We are looking for a highly motivated individual who would like to join our team to deliver good quality service to vulnerable adults.

Able to work flexibly within a 365 day Rota system, which will include evenings, weekend working and bank holidays

Experience of working with customers with mental health needs including - self harm, personality disorder, autism, schizophrenia, or personal experience

Self-motivated and able to work on own initiative

Experience of professional partnership-working

Deliver customer focused support in line with customer hours and service rotas

Ability to create and maintain personalised support plans

Assess risks, write assessments and draw up risk management plans

To regularly hold key worker meetings to review and update customer outcomes

Ability to use Microsoft IT programmes 

NVQ Level 2 in Health and Social Care or working towards

Have own transport.
