Community Mental Health Nurse

Band: 6
Team: CMHT
Working hours: 37.5 hours per week, 09:00 - 17:00 with some flexibility and working from home
Location: Oldham
Pay rate: £26-27 per hour
Must be a Registered Mental Health Nurse with a minimum of 1 year experience

About the team

The CMHT work with people aged 16 and over with severe and eduring mental healh and social care needs. The CMHTs are integrated with the local authority, providing a multi disciplinary approach to the care provided. Care co-ordinators are responsible for provding holistic assessment, interventions, risk assessments and care plans for their caseload.

Main Duties

Assessment of health and social care needs.
Producing, monitoring and evaluating collaborative care plans.
Risk assessment and management.
Providing evidence based theraputic interventions.
Providing education advice, emotional and psychological support.
To promote choice in service user involvement in care planning and risk management.
