Job Summary:
We are seeking a highly skilled and dedicated Clinical Pharmacist to join our healthcare team on a temporary basis. As a Clinical Pharmacist, you will play a vital role in providing exceptional patient care by ensuring the safe and effective use of medications. Your expertise in medication administration, ICU, and anatomy knowledge will be essential in optimizing patient outcomes.
Responsibilities:
- Liaise with Lead Directorate Pharmacist to provide and develop clinical pharmacy services including ward pharmacy and medicines management
- Review and assess medication orders for accuracy, appropriateness, and potential drug interactions
- Collaborate with healthcare professionals to develop and implement individualized medication therapy plans
- Provide medication counseling to patients regarding proper usage, potential side effects, and drug interactions
- Monitor patients' responses to medication therapy and make necessary adjustments to optimize outcomes
- Participate in interdisciplinary rounds and provide clinical recommendations to the healthcare team
- Conduct medication reconciliation to ensure accurate medication histories for patients
- Stay updated with the latest advancements in pharmacotherapy and contribute to evidence-based practice initiatives
Requirements
- Pharmacy Degree MRPharmS
- Registered with GPhC
- 2 years NHS Experience
- Post graduate experience in hospital pharmacy
- Experience in working in an out of hours service
- Experience of working on general medical wards, stroke and elderly care wards
- EPMA (Cerner or other software) experience
