Clinical Pharmacist
Location: West London
Start: Monday 29th July
Permanent, Part-time

Are you a dedicated Clinical Pharmacist looking for a part-time opportunity? We are seeking a skilled individual with Independent Prescribing qualifications to join a practice in West London.

As a Clinical Pharmacist, you will play a key role in supporting Primary Care Networks and member practices. Your responsibilities will include effective medicine management, identifying areas for improvement, and driving positive changes in line with the NHS Long Term Plan.

Main Duties:
• Provide primary support to general practice staff for prescription and medication queries.
• Assist in managing the repeat prescriptions system.
• Handle acute prescription requests.
• Complete medicines reconciliation during care transitions and ensure safer prescribing practices.
• Offer clinical expertise in medicines advice, addressing public and social care needs within GP practices.
• Lead on medicines optimisation, quality improvement, and manage aspects of the quality and outcomes framework and enhanced services.
• Manage chronic disease areas within the practice and conduct clinical medication reviews for patients with complex polypharmacy.
• Ensure integration with community and hospital pharmacies to enhance skill mix, improve
• patient outcomes, and streamline healthcare access.

Skills and Knowledge:
• Strong therapeutic and clinical knowledge with a deep understanding of evidence-based healthcare principles.
• Familiarity with primary care prescribing and strategies for rational prescribing.
• Excellent written and verbal communication skills.
• Proficiency in IT.
• Ability to prioritize tasks effectively.
• Capable of working under pressure and meeting deadlines.
• Ability to work both independently and as part of a team.
• Self-motivated and adaptable.
• Full driving license and willingness to travel as needed.
• Up-to-date mandatory training.

Qualifications:
• Pharmacy degree from a UK university.
• Independent Prescribing Certificate or working towards it.

Experience:
• Minimum of 2 years post-qualification experience in Pharmacy (Hospital, Primary Care, or Community).
• Familiarity with clinical systems such as SystemOne.

Desirable:
• Experience working in a GP practice.
• Independent prescribing qualification.
