I have a great opportunity to work alongside an excellent, close-knit team, with the option to work from home once a week. This flexibility supports a healthy work-life balance. As a Clinical Pharmacist, you will play a vital role in improving the quality of care and operational efficiencies within general practice.
Your role will include but not be limited to:
• Optimise medications for various conditions, recommend improvements, and collaborate with care home staff.
• Conduct medication reviews in homes, suggest changes, and refer patients when necessary.
• Manage minor ailments, offer clinics and telephone support, address medicine-related queries.
• Review medications linked to unplanned admissions, ensure continuous supply post-hospital discharge.
• Refer patients appropriately, manage repeat prescriptions, and identify high-risk patients.
• Advise on new services, analyse medication data, implement changes based on guidelines.
• Monitor prescribing practices, educate healthcare teams, mentor junior members, ensure compliance.
Qualifications:
• GPhC registration as a Pharmacist
• Masters degree in Pharmacy (MPharm) or equivalent.
• CPPE Pathway
• Independent Prescribing Qualification
Ideal candidates will have a minimum of two years' experience in primary care.
